It truly is such a subjective thing. Obviously to give something 100% would be to imply it is absolutely perfect, which to me (once again opinion) is not possible or to imply as Eusis stated that the game achieved it's goal with no perceivable flaws. I would tend to stay away from the perfection idea and go with the idea of no, or little perceivable flaws. But even that is to an extent objective, for instance I think Fallout 3, Borderlands, Valkyria Chronicles, Nier, Persona 4 and Tales of the Abyss are simply the best games I have ever played, but obviously they all have flaws, however, I can overlook the flaws so to me they're probably as close to perfection as I rate anything.

My subjective near-perfect list is as follows....
Seiken Densetsu 3 (Secret of Mana sequel)
Final Fantasy VI
Suikoden 2 (haven't played it in a long time though.... another run-through may change my mind)
Xenoblade Chronicles
Chrono Trigger (I think this one is as close to a verdict as your going to get seeing as how it would pop up on most lists)
Earthbound
Lunar Silver Star Story Complete (many would disagree I'm sure...)
Legend of Heroes TITS
Tales of Symphonia (but right now Graces is giving it a run for its money in my book)
ahhh damn... you know what? None of these are perfect and there are a ton more of equal caliber I would love to list.... so I'm just gonna stop there.....

I would score no game 100%, as I have yet to play a perfect game, and nothing in my opinion really comes close to that.

It won't. Suiko 2 stands the test of time better than any other older-gen RPG I've played in the last 5 years. The high quality sprite based graphics certainly help in that respect.
The only thing that would stop me giving it 100% would be the difficulty, which I think is probably a bit on the easy side.
I'd have given Wizardry 6 a near perfect score when it came out, likewise Pools of Darkness - but neither have aged half as well as S2.

100% overall is a tall order. Heck, 100% in any individual category is a tall order, though I would probably give 100% in music to Ys: Oath in Felghana.
I say this all the time, but I somehow need to play Panzer Dragoon Saga. That game sounds like it would be 95 or above in all categories.

In recent memory, P3P and (visual novel) 999.
Outside of that ... Lunar 2: EB, Xenosaga III, and the PSP remake of FFT all hold up well enough that, on my happier days, I'd award them the "perfect" score.

Zelda: Twlight Princess (I know many will disagree, but I find the game to be flawless, better dungeons than OoT, IMO)
maybe Okami (though it might be more in the 98% area).

My favorite game ever is Morrowind: TES3 (especially GOTY). I know it has problems mainly with combat being stiff, but, the overall feel of the world and shear amount of freedom still blows me away even 10 years later there are few games I think can compare aside from graphics. In ways, I feel Oblivion and even Skyrim took some steps backwards, especially Oblivion. Basically the main things that I really feel should have been kept in Skyrim and Oblivion are spear, unarmored and Levitate aside from a few other knit picks.
For me, its pretty much the RPG I still compare all other RPGs to and its my 100% for now because I still feel in many ways it is unmatched. The PC version did get a 96% here in the reviews and the write up was really detailed and described the game nicely. There are other games I would technically give 100% to, but, when I compare them to TES3 they usually lose out in some category.
Before TES3 came along FF7 was my 100%.
As a few posters mentioned, there is no 100% but, I think the question really is - "if you had to give one RPG 100% which one would it be?"
