Chrono Trigger, which I've played 10-15 times.
Mass Effect 2... or at least, the Renegade options.
Final Fantasy VI and VII.
Super Mario 1, 3, World.

Lufia 2 - I have memorized the solutions to all of the puzzles except one (that being the optional super puzzle)

Resident Evil 2, mostly Scenario A though. I loved this game so much as a kid, and I played and beat it some many times. Still cant do it under 4 hours though, foo.
I don't replay a lot of games, so this is probably my only entry on the list.

Megaman 2, 3 - I used to play 2 all the time, but more recently I play 3 to relax and I have it down pat.
Zelda: Ocarina of Time - Used to play it once per year. I still know where all the heart containers and gold skulltulas are.
Zelda: Twilight Princess - Don't care for the story really, but I love the dungeons and bosses and they keep me coming back for more.
Chrono Trigger - Played through it numerous times.
Fire Emblem - The first one for GBA that is. I pretty much played it non-stop for the first 6 months after its release.
Super Mario Bros. 3 - It took some time, but now I can play through all the worlds with minimum item usage.

SaGa Frontier all scenarios but asellus and lute....though lute's is essentially all sidequests.
FFTactics
Skies of Arcadia though some discoveries do take longer to find than others...
Earthbound
Lufia 2 and Curse of the Sinistrals
Legend of Legaia
Star Ocean 2 any character and all story progression
Grandia 1 and 2 though terminology is slightly fuzzy.
Golden Sun 1 and most of 2
I'm going to call shenanigans on Hathen for PSO cuz unless he's just referring to quests and map completion, he doesn't know every off every enemy for every section id. However, if it's just maps and quests, then yeah, nevermind.
I can sort of say Phantasy Star Universe, but only the online component and only up to about 2008 when I stopped playing. Anything else added would be foreign to me then.
Edit: Oh yeah, every metal gear solid title to date. I can play them with my eyes closed. Actually beat Vulcan Raven doing that once, but it was on easy and I had a lot of claymores.

Embarrassingly large chunks of Morrowind.
EDIT: In high school I had MGS 2 memorized for all intents and purposes. I played that game over and over for dog tags and such. Naturally I didn't find out about the second edition (with VR missions) until much later.

I've also played so much Suikoden 1&2 that I frequently act it out as a voice actor when the words appear in screen.
FF4&6 also come to mind, but its been a few years. All the previous are annual occurances.
Bah I forgot Chrono Trigger and Cross. AND Symphony of the Super Metroid (whenever I feel like these games I tend to just play through both)

When I was a youngun I had Wizards and Warriors down pat and could beat it w/out dying. Now? Can't clear level 1 without cursing my maker.

Ah, back when we had nothing better to do with ourselves but play one of the handful of games we had as a kid. No need to worry about generations, backward compatibility, or needing to replace parts, batteries, controllers, or whatever. And the biggest questions involved whether you did well enough in school to get to rent a game that you might one day hope to finish, and whether you'd get invited over to that kid with the Genesis console or not.
