Im sure this has been done before but I'd be interested to see some posts on this again. I'll start with Ghouls and Ghosts on Genesis. It took me the better part of a year when I was a lad. I know I couldn't do it again.

Probably Devil May Cry 3 on hard mode. I played the original too, not the "easier" remake. I could also say Mega Man 9, as unless you memorized everything in each stage random factors will pwn the shit out of you.

Operation Darkness with no one dying.  Not really hard, just very cheap.

The only thing I can think of right now is Castlevania Rondo of Blood hard mode.  I'm not really sure what the consensus is for difficulty on most games.

EDIT - Thought of some more that gave me trouble: Rengoku, Strange Journey (but that's mostly because of the final boss), and Gitaroo Man (not sure if this is because you can't calibrate it though)

Probably Dracula X for the SNES? Don't play that, guys. It's not great and unnecessarily hard. Play Rondo of Blood or Super Castlevania IV instead.

Probably Xenogears. Which... might have a legitly hard final boss if you did what I did and never got Hypermode for Fei.

My mom did too....yeah, my MOM.  It's weird, she is not a gamer but just that one clicked with her.

I'm gonna say whatever Tri-Ace game.  They tend not to make 'em easy and usually I make a point to go after *everything* in the post-game.  So there's that.

Bayonetta on one of the harder difficulties too.

Kingdom Hearts II Final Mix on a level 1 challenge.

Fighting Terra and the OXIII data fights and finally winning was a process that took weeks, if not a few months. Most challenging and most satisfying victory ever.

EDIT:

Without cheesing the stat-boosting items.

Funny you mention this game. I had the hardest time beating this game too. I literally finished the boss with 1 HP left on one of my characters and was shocked I actually won with my last attack. I beat that game a decade or so ago and still think about it today how I barely beat it... and if I didn't... well... I don't think I would've bothered again with the boss since the fight was waaay long and frustrating for me. I had something similar happen with FFX too, but don't remember the fight being as hard as Xenogears.
