Well, in the case of Ion the game keeps informing you that Ion is a dude, and he's also supposed to be fairly young.  It's a common theme for boys to be voiced by women, since their voice pitches are much higher.  I dunno, it's never been very hard for me to tell women and effeminate men apart.

Wakaba in Suikoden 2 got me though.  I thought she was a boy for the longest time.

I had no idea Tuta from Suikoden II was a little dude until I played III. Also, his master's sprite (Yuan? I can't remember the doc's name) sometimes looked to have bewbies, though I knew he was a guy.

What got me was Milich (was young meaning ignorant and didn't read the text 80% of the time), I thought Milich was girl coz he loved dresses, beauty, and fashion and was shocked to realize he's a man.

Oh here's another one.  In Laxius Force II, I just noticed that Luci refers to her cat Herasia as "him."  I always thought she was a girl cat ever since I got into that series last year.  Although similar sentiments could be said for many animals.

i thought vivi was a boy since zidane referred to him as little guy once in the game...if my memory serves me correctly

Birdo's gender has changed throughout various American game versions, but has always been male in Japan. In the original manual for Super Mario Bros. 2, Birdo is referred to as male that believes he is female, and would rather be called "Birdetta." The Japanese manual says its name is "Catherine", but it would rather be known as "Cassie." Most later American-localized games, such as the Mario sports titles, have usually listed Birdo as a female. Recent localizations have carefully avoided the use of any gender pronouns when describing Birdo. In Super Smash Bros. Brawl, it is said that Birdo is of "indeterminate gender" in its trophy.
