I don't think any game has ever made me sick, but I did happen to catch pneumonia the day BEFORE I picked up Persona 4. The following two weeks however, ended up being the best two week I have ever experienced. Despite having to blow my nose every few minutes, on top of coughing and hacking out phlegm, I enjoyed Persona 4 so much that it took my mind off the fever and the rest of the sickness. I also finished the game 3 times in those two weeks of sickness. It was actually a fun time.

Going back to school and work afterward, was a pain though. :P

FFVI for me.  I was up all night for 2 weeks while taking a course(8 to 5 every day) and I was very sleep deprived which resulted in feeling like complete shit. I wouldn't have changed it for anything though. Still in my top 5 games ever.

Legendary. It gave me a splitting headache throughout the night. That is one of the simple reasons why it's far from legendary.

Also, I got pretty dizzy when playing Mario Kart DS once. But that was in the car so what can you expect?

Actually I just remembered Yoshi's story(I believe) on the SNES. There was one level where if you touched a certain thing the screen would go all melty and swirly and shit. Man that gave me such a headache!
Oh, and when you took a melee hit in Perfect Dark. The blurred vision effect lasted WAYYY too long.

^Pretty much the best stage in Yoshi's Story. I thought that effect was really cool back then.
